NOTE: When a negative TB Skin test is required, a photocopy of the signed document prepared at the time of the TB skin test, must be given to your instructor. If you are unable to get a TB Skin test due to a previous reaction, you must get a chest x-ray and provide a photocopy of the results to your instructor. It is only necessary to have a chest w-ray every 5 years. Prior to the start of your class, you must also have current (within the previous 6 months) written documentation from a physician stating that your are currently free from active disease.
