submitting this form. A staff member will follow-up to discuss the request.  While we can not guarantee that requests will

Note:  The supervisor will be responsible for providing the volunteer/s with training, materials needed for the assignment and instruction.  The supervisor will provide volunteer/s with information on who to contact if questions arise during the assignment.  The volunteer supervisor will submit volunteer timesheets to the Volunteer Center office by the 5th of each month.
